  • Current IVP fingerprint clearance card
  • National Provider Identifier (NPI) number
  • Background or experience as a school-based occupational therapist preferred

  • Assess, plan, and implement individualized therapy programs
  • Collaborate effectively with teachers, families, and other specialists
  • Document student progress in line with school and district policies
  • Support students functional and academic success throughout the school year
