Position Summary

We are seeking a licensed Occupational Therapist (OT) to provide patient-centered therapy services focused on improving residents’ independence and quality of life. This role involves evaluation, treatment planning, interdisciplinary collaboration, and documentation compliance.

Key Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide skilled occupational therapy services per physician orders
• Document evaluations, treatments, and progress notes in compliance with regulations
• Collaborate with physicians, nursing, and interdisciplinary team members
• Educate residents, families, and caregivers on treatment plans and home programs
• Participate in care planning, discharge planning, and team meetings
• Conduct staff training and in-service education
• Maintain compliance with state practice acts and facility policies

Qualifications

• Degree in Occupational Therapy (Bachelor’s required; Master’s or Doctorate preferred)
• Active Occupational Therapist license in South Carolina (in good standing)
• Experience in skilled nursing or rehabilitation setting preferred
• Familiarity with PointClickCare and Casamba preferred
• Strong clinical, documentation, and communication skills
