Overview This is a Fee for Service Position on Team 2 - Catholic Health Home Care is dedicated to providing compassionate, high-quality home care services to patients across Long Island. As part of our mission, we are seeking an experienced and caring Occupational Therapist (OT) to join our team. You will have the opportunity to make a positive impact on the lives of patients in the comfort of their own homes while collaborating with an experienced and supportive healthcare team. As an Occupational Therapist Team 2 you will work in multiple locations including: Atlantic Beach, Baldwin, Cedarhurst, East Rockaway, Bellmore, Freeport, Hewlitt, Inwood, Island Park, Lawrence, Levittown, Lido Beach, Long Beach, Lynbrook, Merrick, North Bellmore, North Merrick, Oceanside, Point Lookout, Rockville Center , Valley Stream, Wantagh, Woodmere. Job Details Key Responsibilities: Conduct assessments of patients’ physical, cognitive, and emotional needs in their home environment. Develop and implement personalized treatment plans to improve patients’ ability to perform daily activities. Work collaboratively with other healthcare providers, including physicians, nurses, and home health aides, to deliver coordinated care. Educate patients and families on therapeutic techniques, adaptive equipment, and ways to enhance the home environment for safety and independence. Document patient progress, treatments, and goals according to regulatory requirements. Participate in team meetings to discuss patient progress and treatment plans. Salary Range USD $90,000.00 - USD $115,000.00 /Yr. Qualifications: Education: Bachelor’s or Master’s degree in Occupational Therapy from an accredited program. Licensure: Active and valid Occupational Therapist license in New York State. Experience: Prior experience in home care is preferred but not required. New grads are welcome to apply. Skills: Strong interpersonal, communication, and problem-solving skills. Ability to work independently while managing patient caseload. Other: Reliable transportation to travel to multiple locations in the designated area.
