About Santa Rosa Post Acute

Santa Rosa Post Acute is a premier skilled nursing and rehabilitation facility dedicated to providing exceptional post-acute care and rehabilitation services. Our therapy department partners closely with nursing, physicians, and case management to create individualized treatment plans that help residents achieve their highest level of independence and quality of life.

Responsibilities

  • Evaluate residents and develop individualized occupational therapy treatment plans
  • Provide skilled occupational therapy interventions in accordance with physician orders and established care plans
  • Help patients improve performance in activities of daily living (ADLs), self-care tasks, mobility, cognition, and functional independence
  • Collaborate with Physical Therapists, Speech Therapists, nursing staff, physicians, and families to coordinate care
  • Complete timely and accurate documentation in compliance with regulatory and facility requirements
  • Participate in interdisciplinary team meetings, care conferences, and discharge planning
  • Educate patients, families, and caregivers regarding therapeutic techniques and home programs
  • Maintain compliance with all federal, state, and professional standards of practice

Qualifications

  • Current California Occupational Therapist (OT) license in good standing
  • Graduate of an accredited Occupational Therapy program
  • Skilled nursing, post-acute, rehabilitation, or geriatric experience preferred
  • New graduates with strong clinical rotations are encouraged to apply
  • Experience with Casamba, Net Health, PointClickCare, or similar EMR systems is a plus
  • Strong communication, organizational, and teamwork skills
  • Passion for helping patients achieve their highest level of function and independence
