Midtown Oaks Post Acute – Sacramento, CA
$60–$65/hour + $7,500 Sign-On Bonus

Midtown Oaks Post Acute is seeking a compassionate and motivated Full-Time Occupational Therapist (OT) to join our skilled nursing and rehabilitation team in Sacramento, CA.

Occupational Therapist (OT) Responsibilities

  • Evaluate residents and create individualized occupational therapy treatment plans
  • Provide skilled OT treatments to improve mobility, function, and independence
  • Collaborate with interdisciplinary teams including nursing, therapy, and physicians
  • Educate patients and families on treatment plans and recovery strategies
  • Maintain accurate documentation in compliance with state and federal regulations
  • Monitor patient progress and adjust treatment plans as needed

Occupational Therapist (OT) Qualifications

  • Active California Occupational Therapist (OT) license
  • Strong clinical and patient care skills
  • Excellent communication and teamwork abilities
  • Skilled nursing facility (SNF) or post-acute experience preferred, but new grads are welcome to apply
  • Passion for helping geriatric and rehabilitation patients achieve success
