Concept Rehab is seeking a Certified Occupational Therapy Assistant (COTA / OTA) to join our team at a skilled nursing facility (SNF) in Avon, Ohio. What You’ll Do
As a Certified Occupational Therapy Assistant in a Skilled Nursing Facility, you will help patients improve independence with daily living skills following illness, injury, or hospitalization.
Key Responsibilities:
- Deliver occupational therapy treatments under the supervision of an OTR
- Implement individualized care plans focused on ADLs and functional independence
- Collaborate with therapists, nursing staff, and families
- Maintain accurate and timely electronic documentation
- Educate patients, caregivers, and staff on therapy programs
- Assist with discharge planning and patient progress tracking

Requirements
Qualifications
- Graduate of an accredited Occupational Therapy Assistant program
- Active Ohio COTA license (or eligible)
- Strong communication and teamwork skills
- Passion for working with geriatric and rehab populations
- New graduates welcome to apply
Preferred Experience
- Skilled Nursing Facility (SNF), rehabilitation, or post-acute care experience
- Experience with EMR/EHR documentation systems
Ideal candidates may have experience in Skilled Nursing Facilities (SNF), Long-Term Care (LTC), Post-Acute Rehabilitation, Assisted Living, Home Health, or Outpatient Therapy settings. New graduates are encouraged to apply.
